About this course

Across all sectors of engineering, including oil and gas, mining, utilities, defence and manufacturing, the design of systems and equipment is becoming more complex. Failures of this equipment are becoming are becoming more acute and complex requiring engineers to understand that the root cause or causes do not just stop at the the physical reason why assets fail but also examine human root causes and systemic root causes that may all lead up to the physical failure of the asset.

This course will introduce you Root Cause Analysis from the simple 5- Why’s approach building up the comprehensive logic tree analysis approach explaining prerequisites and the Pros and Cons of each technique.

As a participant, you will be able to understand which form of Root Cause Analysis may be adequate to address the magnitude of the failure of a process, equipment and/ or system, and use the appropriate RCA process to identify then rectify or eliminate the failure experienced.
